Earn $5 per case on up to 100 cases, for a maximum rebate of $500 total. Purchases qualify when made between October 1, 2024 and May 31, 2026.

How It Works — 3 Easy Steps

• Purchase eligible CJ Schwan's products from a single distributor between October 1, 2024 and May 31, 2026.
• Complete the rebate form — fill in your product quantities, total rebate, and contact information.
• Submit your form with your distributor invoice(s) by mail or online — no later than June 30, 2026. Then sit back and collect your rebate.

What You'll Need to Provide

• The products & case counts: distributor code, product code, and number of cases purchased for each eligible item.
• Your total rebate requested: $5 × total eligible cases (up to $500).
• Business details: business name, contact name & title, street address (no P.O. boxes), city, state, ZIP, phone, and email.
• Distributor info: your primary distributor, sales rep/broker, and their city and state.
• Operation type: check the box that best describes your business (casual dining, catering, hospital, etc.).
• Proof of purchase: your distributor invoice(s) with the eligible products circled.
